  2. The names of the parties at the time the case was filed
  3. The specific documents you want to receive
  4. The filing date or year filed
  5. Your phone number and e-mail address so they may contact you

By the way, for example in California (www.cacd.uscourts.gov) are some documents eligible for SmartScan. This is a paid service. Remember, SmartScan service request is limited to a maximum of 100 pages. The SmartScan request may take one to seven business days, according to the FRC. Its called "Public Access to Court Electronic Records"
